Tesamorelin for Deep Belly Fat: Patient Stories and the Trial Results

People report striking changes, subtle changes, and side effects with tesamorelin. Human trials show a narrower, measurable benefit in a specific group.

Abstract cross-section with amber shapes representing fat deep within the abdomen

Tesamorelin can reduce fat stored deep inside the abdomen in adults with HIV-related fat accumulation. That result comes from randomized human trials, not only online stories. It does not mean that every person with a soft lower belly will see a dramatic change in the mirror. Reviews describe real relief, disappointing results, and side effects. Reading them alongside the research makes both more useful.

Tesamorelin is a manufactured version of a signal that tells the pituitary gland to release growth hormone. The US prescription product Egrifta WR is approved to reduce excess abdominal fat in adults with HIV and lipodystrophy, a change in body fat distribution. Its official label says it is not indicated for weight loss management. That distinction matters to anyone arriving here after searching for tesamorelin belly fat results.

What patients say happened

One patient review of Egrifta describes losing several inches around the abdomen after beginning treatment in 2011. The reviewer says bending over and seeing their feet became easier. Another reviewer describes a visible improvement in chest shape and self-esteem during 18 months of use, followed by returning fat after stopping because of cost. Their words capture something a scan cannot: how a changed body shape can affect daily comfort and confidence.

The same review page contains people who saw little benefit. One says three years of injections did nothing noticeable. A different reviewer describes gaining weight over eight months. Another reports a smaller waist and lower weight but also severe joint pain soon after starting, with symptoms returning after trying the medicine again. These posts cannot establish a drug reaction on their own, yet they give a prospective reader concrete reasons to discuss symptoms with a clinician. A review site selects for people motivated to post, and its small group of reviewers is no substitute for the trials.

Reddit accounts add a different population. In one discussion about a five-week deadline, a commenter says their waist changed after a much longer course, while another reports little weight loss after eight months. The original poster had already lost weight and was asking about remaining lower-belly fat. That is not the condition tested in the main HIV trials. Someone describing a two-inch waist change may be sincere, but a tape measure cannot say how much of that change was deep fat, fat under the skin, bloating, or a concurrent change in food and activity.

Another Reddit account combining retatrutide and tesamorelin describes a substantial weight change and better sleep and recovery. The writer also lists several other peptides. Retatrutide, diet, training, and the other products make it impossible to assign the outcome to tesamorelin. A separate combination account praises changes in the lower belly while also adjusting retatrutide. These are interesting personal timelines, not evidence that two peptides work better together.

First-person videos offer another way to hear what use feels like. A 30-day video diary says its creator checked blood work and a body-composition scan. A 12-week tesamorelin and ipamorelin video describes a combined experiment and a decision to stop early. The available descriptions do not establish verified scan results or the reason for stopping, so this article does not assign outcomes to either video. A viewer should check the actual measurements, dates, other treatments, and any sponsor disclosures before treating a video as a result.

What 30 Reddit users reported

These are first-person accounts, quoted in the writers’ own words and linked to their individual Reddit comments. Spelling and wording inside quotation marks are preserved. The usernames and outcomes have not been independently verified. Many people used another peptide or medicine, changed diet or exercise, or relied on a home body-composition device. Their experiences can help frame questions, but they cannot establish how often tesamorelin works or causes a side effect.

Waist and appearance

u/tammytumms wrote: “I have never in my life had a flat stomach and I at one point had a POOF, BUT now that's even gone.” The commenter also describes being an older woman who noticed a change after several weeks. No before-and-after measurements or scan are posted in the comment. Read the original Reddit comment.

u/Desperate_Ferret2744 wrote: “Have lost 4” off my waist and can see the difference in the belly area.” The writer reports seven weeks of tesamorelin while also taking retatrutide for fourteen weeks. The waist change is concrete, but the comment cannot separate the effects of the two products. Read the original Reddit comment.

u/SnooStrawberries2358 wrote: “For me, it 100% helped trim down my waist and mid section, takes a few weeks but muscles feel fuller, just feel better overall.” This 57-year-old commenter also reports fluid retention and says there was no baseline IGF-1 test. The experience is positive but the post offers no independent body-fat measurement. Read the original Reddit comment.

u/Reasonable-Echidna67 wrote: “Tesamorelin is amazing. 2mg bedtime after 8 weeks my pants were loose on me.” The commenter also says their diet did not change. Looser trousers are a useful personal observation, but they cannot identify whether deep or under-skin fat changed. Read the original Reddit comment.

u/Knicco wrote: “I went from almost a size 32 to 29.” The same person explicitly says they did not use tesamorelin alone and describes stacking it with ipamorelin. The waist-size report therefore cannot show what tesamorelin alone did. Read the original Reddit comment.

u/Hamamellis_FTW wrote: “I am like a "hyper-responder" to tesamorelin.” The user says their diet and training also have to be in place and describes a strong subjective fat-loss response. There is no scan or controlled comparison attached to this account. Read the original Reddit comment.

u/Lizzy_is_a_mess wrote: “I took it for 12 weeks and only on week 14 did it start working and overnight it seemed I lost 2 inches.” The writer describes a delayed waist change. The phrase “it seemed” matters because a self-measured circumference can shift with posture, bloating, and other changes. Read the original Reddit comment.

u/Angela_MAGA_OSU-1 wrote: “However, it took 8 weeks for me to actually see the results.” The commenter attributes body-fat changes to a tesamorelin and ipamorelin mix and also mentions working out. The timeline is firsthand; the cause of the change cannot be isolated from the post. Read the original Reddit comment.

u/Nadia_sapphire wrote: “I’m on my 6th week of Tessa and tummy starting to look more defined.” The writer describes a visible change after six weeks. No scan or independent waist measurement is supplied, and the comment does not establish whether the change was visceral fat. Read the original Reddit comment.

u/Visual-Geologist-895 wrote: “Since starting I've lost 3" around my waist, lost over 16lbs of fat, visceral fat score went from 11 down to 7.” The same user says they lifted weights daily and also used ipamorelin and retatrutide. The body-composition score came from an InBody device, not the CT scan used in the major trials. Read the original Reddit comment.

u/DistributionSalty751 wrote: “So far I've lost 6 inches around my waist.” This person says they were also using retatrutide and planned a DEXA scan later. Their belief about which product caused the waist change was not yet confirmed by the scan in this comment. Read the original Reddit comment.

u/bauer131 wrote: “I just started Tesa a couple weeks ago and admittedly haven’t been as consistent with it at night as I need to be but I’ve definitely seen some results.” The commenter later says they were also on testosterone treatment and had switched from sermorelin. They describe less belly fullness, with no separate measurement of fat or bloating. Read the original Reddit comment.

u/Mrnightmarechaser2 wrote: “I took measurements at my navel throughout the cycle. I started with a measurement of 44” my measurement this morning was 38”.” This commenter describes a ten-week course and also mentions arm cramping. The before-and-after waist figures are self-reported, without a scan or comparison group. Read the original Reddit comment.

Mixed or uncertain results

u/Personal_Nebula_7100 wrote: “Made no difference at all.” That was the user’s description of an initial eight-week period with tesamorelin and ipamorelin. They later reported abdominal definition after changing their use, while saying they were unsure whether the products caused it. Read the original Reddit comment.

u/fauxzempic wrote: “So far, I've only been using tesa + Ipa for 2 weeks, and all that's happened is a ballooning in water weight, but I know that effects will take at least 10-12 weeks just to start.” This early account reports fluid or scale change rather than visible fat loss. The writer was using two peptides, and the timing is much shorter than the main clinical trials. Read the original Reddit comment.

u/lundybird wrote: “Straight from the manufacturer Tesa for 8 months here. Lost 4 pounds.” The commenter contrasts that small scale change with much larger weight loss after starting retatrutide. Body weight is not the same outcome as scanned visceral-fat area, but this is a clear report of disappointment. Read the original Reddit comment.

u/No-Astronomer-6808 wrote: “I’ve been on Reta for almost 5 months and Tesa for almost 5 weeks. In my experience, the biggest changes have come from Reta.” The commenter also says pinchable lower-belly fat remained. Their account distinguishes a visible cosmetic goal from the deep-fat measurement used in tesamorelin trials. Read the original Reddit comment.

u/kod138 wrote: “Yes, but I started a lot of things at once because I am impatient .” The user then lists testosterone treatment, retatrutide, ipamorelin, exercise, and a calorie deficit alongside tesamorelin. Their home-scale body-fat trend is real to them but cannot establish which change drove it. Read the original Reddit comment.

u/Civil_Practice9386 wrote: “I have gained 10 lbs since then. Hoping it’s just water weight because I’m taking it all to lose weight and look more fit. Kinda bloated right now.” The commenter had added tesamorelin to retatrutide and a CJC-1295/ipamorelin combination three weeks earlier. The cause and composition of the weight increase were unknown. Read the original Reddit comment.

u/Professional-Tell790 wrote: “The water retention is annoying af. My waist has leaned out but didn’t see any crazy results.” The user was combining tesamorelin and ipamorelin, running regularly, and lifting weights. They reported some waist change while describing the overall result as less impressive than expected. Read the original Reddit comment.

u/Quirky_Yogurt_6896 wrote: “Finished a 10 week cycle of Tesa and don’t notice anything significant.” The commenter also felt any change might not last after stopping. No scan or other outcome measure appears in the post, but the lack of a noticeable effect is part of the range readers should see. Read the original Reddit comment.

u/noreaster97290 wrote: “I had great results on Tirz and Tesa.” The user also mentions some fluid retention, a reported decrease in visceral fat, and little sleep benefit before switching treatments. Tirzepatide was used at the same time, and the post does not specify how deep fat was measured. Read the original Reddit comment.

Sleep and recovery

u/steavis77 wrote: “I do it just before bed, fasted for 3 hours and it’s helped my sleep as well.” This user describes better sleep, but gives no sleep measurements or comparison with a time off the peptide. Read the original Reddit comment.

u/Repulsive_Trust5895 wrote: “In my case Tesa before bed was bad for sleep: I fell asleep fine (as I always do), but would wake up 4-5 hours later and be unable to go back to sleep.” This is a different sleep experience in the same discussion. The user changed timing afterward, so their account should not be read as a predictable effect for everyone. Read the original Reddit comment.

u/Theappache10 wrote: “For me it disrupts my sleep i like ipamorelin more for sleep As for fat loss i always bloat on it but when i stop it i can see the fat loss difference” The original comment is a single run-on sentence. The user describes sleep disruption and bloating during use, then a perceived appearance difference after stopping; no measurements are supplied. Read the original Reddit comment.

Side effects and stopping

u/backpackleck wrote: “Started at 1mg 5x/week had great results within 3 weeks but joint pain became unbearable.” The same comment describes changing the regimen because of pain. It illustrates why a favorable appearance report can coexist with a reason to stop or seek medical advice. Read the original Reddit comment.

u/my_religion_is_love wrote: “I now have elbow tendinitis (golfers and sometimes tennis elbow) that only started after I introduced tesamorelin. Stopping the peptide did not reverse the pain either.” This is a report of timing and persistence, not a confirmed diagnosis of a drug reaction. The complaint deserves attention because joint symptoms are also described in the product label. Read the original Reddit comment.

u/Dull_Wolverine2538 wrote: “Pushing a door open hurts, and i have to take it easy in the gym now.” The writer says the elbow pain began during their first week of tesamorelin while they were already using retatrutide. They had used ipamorelin before without this symptom. Read the original Reddit comment.

u/ldm147 wrote: “I've experimented with Tesa, Ipa, and Reta in the last few months. Along the way, I developed arthritis like pain in me left hand and tennis elbow symptoms in my right elbow.” The person says the symptoms remained after stopping all three products and was unsure which one, if any, caused them. This is a combined-use account, not evidence against one product alone. Read the original Reddit comment.

u/Wooden_Direction_241 wrote: “The known fluid retention in the joints just would not go away and it hurt like crazy to squat.” The commenter says they stopped tesamorelin because of knee discomfort and had pre-existing knee concerns. “Fluid retention” is their explanation, not a medically confirmed cause of the pain. Read the original Reddit comment.

These posts describe visible changes, no visible change, sleep effects, and symptoms that led people to stop. A photograph, scale, or waist measurement cannot tell how much deep abdominal fat changed. Product identity and other treatments also remain uncertain in many posts. The controlled trials below provide a more dependable estimate for the people those trials enrolled.

Deep fat and the fat you can pinch are different

Visceral fat lies around internal organs, behind the abdominal muscles. Subcutaneous fat sits beneath the skin and can usually be pinched. Both contribute to a waistline, and a mirror cannot reliably separate them. Trials of tesamorelin used CT or MRI scans to measure the deep compartment. They did not test a promise to erase love handles or a particular lower-belly fold.

The 2010 randomized trial with a safety extension followed 404 adults with HIV and excess abdominal fat. For the first six months, participants were assigned to tesamorelin or an identical-looking placebo, meaning an injection without the active medicine. This comparison helps separate a drug effect from changes that occur with time or expectation. The main outcome was visceral fat measured by imaging, rather than a photograph or scale weight.

At six months, visceral fat fell by about 10.9% in the tesamorelin group and about 0.6% in the placebo group. People taking tesamorelin also reported less distress about belly appearance on average. The study did not find a comparable reduction in fat just under the abdominal skin. Those results explain why a person may have less internal fat without seeing the specific cosmetic change they hoped for.

The second part of that trial gives a practical caution. Participants who continued tesamorelin maintained a larger reduction at one year. Those switched from tesamorelin to placebo rapidly lost the initial gain in visceral-fat reduction. The effect depended on ongoing treatment during this study. It did not show that a short course permanently resets body composition.

An earlier randomized trial of 412 adults with HIV also measured deep abdominal fat after 26 weeks. The tesamorelin group had an average decrease of 27.8 square centimeters on the scan slice, while the placebo group had an average increase of 5.1. A square centimeter on one scan slice is a way to measure an area, not pounds of whole-body weight loss. The study supports a specific effect on this fat compartment; it does not prove fewer heart attacks or longer life.

Does the evidence apply outside HIV care?

The research is broader than one diagnosis, but still narrower than social media claims. A 2012 randomized trial enrolled 60 people with abdominal obesity and lower growth-hormone secretion who did not have the HIV-related condition. Over 12 months, tesamorelin reduced measured visceral fat more than placebo, with a between-group difference of about 35 square centimeters. Fat beneath the skin did not show a clear difference. Triglycerides and a few markers related to cardiovascular risk also changed. A marker is a clue about risk, not a demonstration that a treatment prevents a heart attack.

Sixty selected volunteers are not every adult hoping to change their waist. The trial did not establish an approved general weight-loss use or a long-term safety plan. People with ordinary weight gain may have different hormone levels, risks, and expectations. Online advice that applies the study to everyone skips those differences.

The 2014 six-month trial studied 50 adults with HIV and abdominal fat accumulation. Tesamorelin reduced both measured visceral fat and liver fat more than placebo. The liver-fat difference was modest, and the researchers called for more work on what it means for long-term health. Glucose rose briefly early in the tesamorelin group, although the main glucose comparisons at six months were not statistically clear. That is a reason for monitoring, not for claiming that the peptide improves or worsens blood sugar in every patient.

A later one-year trial in 61 people with HIV and fatty liver found a larger decrease in liver fat with tesamorelin than placebo. It also reported less progression of liver scarring in the treated group. Liver fat was measured by magnetic-resonance methods, and tissue samples provided additional information. The participants had a defined liver condition. The result does not prove that tesamorelin treats every kind of fatty liver, and the trial was too small and short to settle lifelong outcomes.

Why a positive story still needs context

Online posts can answer questions that trials rarely capture: Was the routine burdensome? Did someone feel comfortable in clothes again? Did swelling or pain interfere with life? They cannot establish product identity, control for simultaneous changes, or reveal how many people had no result and never posted. A before-and-after image is particularly hard to interpret when body position, lighting, diet, and other drugs changed.

The clearest success stories on prescription Egrifta concern people with HIV-related body changes, the population for which the strongest trials were designed. Many peptide-forum stories concern bodybuilding or a blend with ipamorelin, retatrutide, or other agents. A blend is a different experiment. The site's discussion of peptide combinations explains why an appealing combined story cannot establish which ingredient helped.

Safety and product identity

The current US label calls for attention to blood glucose and IGF-1, a growth-related blood marker. It lists swelling, joint and muscle pain, injection-site reactions, and possible allergic reactions. It says people with active cancer, pregnancy, or certain pituitary problems should not use the product. Long-term cardiovascular safety has not been established. A person with chest symptoms, trouble breathing, or a severe allergic reaction needs prompt medical care rather than advice from a forum.

The same label distinguishes Egrifta WR from Egrifta SV. They have different formulations and instructions and are not interchangeable. A vial sold online as research tesamorelin is not automatically either approved product. Its name does not verify what is inside, how much is inside, or whether it is sterile. Trial findings belong to the studied medicine and conditions, not every item bearing the peptide name.

For someone with HIV-related lipodystrophy, the useful next question is whether deep abdominal fat is actually the concern and how benefits and monitoring fit their health history. For someone seeking general cosmetic fat loss, the evidence gives a much less direct answer. Patient stories deserve attention because bodies and daily life matter. The scans tell us which part of those stories the controlled studies have truly tested.
